It is easier than ever to get the physical therapy you need. Merz Physical Therapy is a Medicare certified private practice clinic able to submit claims on your behalf to most insurance carriers. You can access our services in the following ways:

1. Direct access
Minnesota law guarantees a patient's direct access to physical therapists without a doctor referral for 30 consecutive days of care. After which the patient must receive a medical diagnosis from their physician to resume care. All members of The Marsh may receive a half-hour consultation from a Merz physical therapist.

2. Physician referral
You can bring a referral form directly to your physician or we can fax a referral form directly to your clinic. Forms are available in our office or online. Click here to download a printable PDF of our referral form (Requires Adobe Acrobat Reader).

We accept most healthcare plans as either "in-network" providers or "out-of-network" providers, no-fault auto insurance cases and workers' compensation.
